Assumption of Risk
I, {name}, understand that Brazilian Jiu Jitsu is a physical contact sport that involves risk of injury. I acknowledge that by participating in classes, open mats, and other activities at Midway Jiu Jitsu, I may be exposed to the risk of serious injury, including but not limited to bruises, sprains, strains, joint injuries, or more severe outcomes.

I accept full responsibility for any injuries I may sustain and agree to train at my own risk.

I understand and accept the risks involved in training Jiu Jitsu.

Health and Medical Disclosure
I confirm that I am in good physical condition and have no known medical conditions that would prevent me from safely participating in physical activity. I agree to inform Midway Jiu Jitsu staff of any changes to my health or any relevant conditions that may affect my ability to train.

I confirm I am medically fit to participate.

Release of Liability
By signing this agreement, I release and hold harmless Midway Jiu Jitsu, its coaches, staff, and volunteers from any and all liability for injuries, losses, or damages that may occur as a result of participation, whether caused by negligence or otherwise.

I release Midway Jiu Jitsu from liability for any injuries sustained during training.

Photography & Media Consent
I give permission for Midway Jiu Jitsu to use photos or videos taken during training for promotional or educational purposes.

Parental Consent (if under 18)
If the participant is under 18 years old, a parent or guardian must sign below to provide consent. I, , am the parent/legal guardian of the participant and give full permission for their involvement in all activities at Midway Jiu Jitsu. I agree to all terms listed above.
